If your curious on why only in the US for me, here is some info on the logistics of international tax and laws:

Tax Threshold was lowered or removed for most countries and a ruling in the US changed in 2018:
Here is Big Cartel covering the change in 2018.
Esty has a lot covering selling to other countries.
Here is the one about selling to Japan (the consumption tax)
And about the packaing law in Germany called Verpackungsgesetz.

Refunds:

I will do my best to make the customer happy. I do refunds minus the Paypal fees until I print the product or in case of pre-orders until I place the order. Once the order is placed/or printed up I do not do refunds.
